How do I use this page?
This page provides a list of codes (i.e. AON12, IVL12, DVW20, etc.) outlined in the Financial Aid Notification letter you received from the Financial Aid Office.
Step 1: Refer to your letter, find your code(s) and click on the code(s) below that match those in your letter.
Step 2: Read and follow the directions provided in order to provide required information for your financial aid file.
If you should lose this message, you can still access what you need to know in your MyOTC account online at http://www.otc.edu:
- Click on Connect MyOTC and login
- Click on Access OTC tab at the top
- Click on Notifications under Financial Aid heading

ADAA12 - Anti-Drug Abuse Act
The Department of Education cannot process your FAFSA because of issues related to the Anti-Drug Abuse Act of 1988. To address these issues, you must contact the Department of Education by telephone at 202-377-3889.
AON12 - Aid Overaward Notice
- Your FAFSA results indicate you have a previous overpayment of student aid from the Federal Pell Grant, Federal Stafford Student Loan Program, Federal SEOG or Federal Perkins Loan Programs.
- To resolve your Federal overpayment, call the U.S. Department of Education at 1-800-621-3115, or write to the U.S. Department of Education, P.O. Box 5609, Greenville, Texas 75403-5609.
- You may visit the NSLDS website to review your overaward payment by clicking here.
BAD12 - Aid Denied/SAP Status
- The College is unable to award financial aid at this time because our records show you have not met OTC Financial Aid Satisfactory Academic Progress (SAP) standards. View the Financial Aid SAP Policy for further information by clicking here.
If you believe your inability to meet SAP standards is appropriate for review, please click Financial Aid Appeal Information. You may contact Student Services in person, by phone at 417.447.6900, or email financialaid@otc.edu and include in the subject line Appeal Form.
BDF12 - Bachelor Degree Received
- According to the information you reported on the FAFSA, you have completed a Bachelor's degree. Therefore, you are ineligible for the Federal Pell grant.
If you completed this item on the FAFSA in error, you may update your FAFSA online. If you have completed a Bachelor's Degree and desire to receive student aid, please visit this link for Financial Aid Appeal Information. You may contact Student Services in person, or email financialaid@otc.edu and include in the subject line BACHELOR DEGREE for assistance.
BNK12 - Active Bankruptcy Status
COC12 - Eligible Non-Citizen Status
The Department of Homeland Security (DHS) and/or Social Security Administration (SSA) were unable to confirm your eligible non-citizen status or U.S. Citizenship. You must bring an acceptable document to one of OTC's locations which confirms you are an eligible noncitizen or a U.S. Citizen. For a list of acceptable documents click here.
CSI12 - Cash, Savings and Investment Form
- Completed and signed Cash, Savings & Investments form.

CSP12 - Child Support Paid
- Completed and signed Child Support Paid form.

DFMR12 - Dependent Family Member Review
Review the number of family members you have reported in Item 72 on the FAFSA by clicking here and either confirm your answer by re-entering it or make the necessary correction.
DIR12 - Dependent Income Review
You reported your parents will not file an income tax return, but the amount you reported for your parents’ income appears to be over the minimum amount required to file a tax return. Please review Items 79, 86, and 87 on the FAFSA by clicking here and make the necessary corrections.
DSD - Dependency Status Documents
Your dependency status on the FAFSA requires validation. Based upon your response on the FAFSA, you answered yes to one or more of the following conditions:
- Are both your parents deceased or were you in foster care or ward of the court since you turned age 13?
- You are an emancipated minor as determined by a court in your state of legal residence.
- You are in legal guardianship as determined by a court in your state of legal residence.
In order to verify your response, OTC's Financial Aid Office requires a legal court document that validates your status.

DVL12 - Dependent Verification Request
Dependent Verification Notice and Instructions
Your 2012-2013 Free Application for Federal Student Aid (FAFSA) has been selected for a review process called verification by the U.S. Department of Education. You have been classified as a dependent student based upon information you provided on your FAFSA. The law says that before awarding Federal Student Aid, we may ask you to confirm the information you and your parents reported on your FAFSA. To verify you provided correct information the financial aid office will compare your FAFSA with the information on any required worksheets or documents.
INSTRUCTIONS:
If required, you and at least one parent must compete and sign any requested worksheets, attach any required documents, and submit required forms to the Financial Aid Office. The Financial Aid Office may ask for additional information. If you have questions about verification, contact Student Services as soon as possible so your financial aid will not be delayed.
In Order to most efficiently process your file, any documents submitted to the Financial Aid Office must be submitted at the same time and include your OTC student ID# and name on each page. If you are mailing or bringing the documents to the college, please DO NOT staple the documents together. You may submit the documents:
- In person at any of the College's locations
- By fax to 417.447.6938
- Via email to financialaid@otc edu with the your OTC ID# in the subject line
- Postal mail with Attention to Financial Aid at 1001 East Chestnut Expressway, Springfield, MO 65802
Once documents are submitted please allow 10 business days to view updates on your notifications. After 10 business days, if you are still receiving financial aid notifications on the item(s) you submitted, please follow up. A missing signature, etc., could be the delay. It is your responsibility to ensure all required information for your file is received by the Financial Aid Office.

HCV12 - Household and College Verification
- Completed and signed Household and College Verification form.

HMLS12 - Homeless Status Document
Your dependency status on the FAFSA requires validation. Based upon your response on the FAFSA, you answered yes to 'you were determined as an accompanied youth who was homeless or were self-supporting and at risk of being homeless on or after July 1, 2011'. Review the type of agency contacts below and submit documentation from the agency which was your point of contact for your situation.
- Your high school or school district homeless liaison.
- The director of an emergency shelter of transitional housing program funded by the U.S. Dept. of Housing and Urban Development.
- The director of a runaway or homeless youth basic center or transitional living program.
HSG - High School Transcript or GED Scores
The College does not have an official high school transcript or GED on file for you. In order to be eligible for Federal Student Aid, you must have completed a high school diploma or GED. If you have completed this requirement, send your final GED scores or have your high school send your official transcript to the OTC Admissions Office. If you have questions please contact the Admissions office in person at any of the college's locations, phone at 417.447.6900 or via email to admissions@otc.edu.
IDW12 - Illegal Drug Worksheet
- You are not eligible for federal financial aid based on your response to the question regarding a drug conviction.
You reported in Item 23 on the FAFSA that you are not eligible for federal student aid as a result of a drug-related conviction, or you do not know if your conviction(s) affect your eligibility. However, you may still be eligible to receive state, school, or other non-federal student aid. If you have answered this question incorrectly, you must correct Item 23 on the FAFSA by clicking here. You can also correct this item or get help with this question by calling 1-800-4-FED-AID (1-800-433-3243).
IFMR12 - Independent Family Member Review
Review the number of family members you have reported for Item 93 on your FAFSA by clicking here and either confirm your answer by re-entering it or make the necessary correction.
IIR12 - Independent Income Review
You reported you will not file an income tax return, but the amount you reported for you, and if married, your spouse's income appears to be over the minimum amount required to file a tax return. Please review Items 32, 38, and 39 on yourFAFSA by clicking here and make the necessary corrections.

NDS12 - Non-Degree Seeking
- Records at OTC indicate you are not seeking a degree or certificate.
Federal regulations require a student to be seeking a degree or certificate in order to be eligible to receive Federal Student Aid. You may declare a degree or certificate by completing the Program Update/Change form and submitting it to the college. Click here to go to the form. Once you have updated your program of study notify us via email financialaid@otc.edu with the email subject line DEGREE SEEKING - OTC ID# so your financial aid file can be updated accordingly. Failure to do so will delay processing of your financial aid.
PAE12 - A+ Stamped Transcript Needed
Your A+ High School transcript is not on file. You indicated on your OTC Admissions application you are an eligible A+ student who graduated from a Missouri A+ high school. In order to validate your A+ Program eligibility, the college is required to have your final official A+ designated high school transcript on file. Please contact the Missouri high school you graduated from and request the official A+ designated transcript be sent to the OTC Admissions Office. If you graduated from high school before 2006, did not graduate from a Missouri high school, or you are otherwise not an A+ Program student, please contact Student Services at any of the OTC campus sites for assistance with updating your records accordingly.
PAF12 - Potential Aid Fraud
When the Department of Education processed your FAFSA it was discovered you have one or more student loans which may have been obtained fraudulently. Therefore, you are not eligible to receive any federal student aid until this issue is resolved. You are encouraged to contact the Federal Student Aid Center at 1.800.4FED.AID (1.800.433.3243) for assistance.
PFP12 - Failed Academic Plan
Your financial aid file reflects a prior financial aid probation, appeal, and failed academic plan. Therefore, you are not eligible for Federal Student Aid at OTC due to your prior academic history. More information is available online regarding the college's Satisfactory Academic Progress Policy for Federal Student Aid.

PSM12 - Parent Signature Missing
- According to the FAFSA results received, your parent(s) did not sign your FAFSA or any corrections submitted.
You must have at least one parent sign your FAFSA using their parent FAFSA PIN# online at http://www.fafsa.ed.gov/ using their parent FAFSA PIN or by signing the paper Student Aid Report (SAR).

PUI12 - Parent Untaxed Income
You must provide your parent(s)' income information on the FAFSA in Items 83 through 87 and 92a through 92i.
RFS12 - Received Food Stamps
Submit written confirmation showing you received food stamp benefits in either 2010 or 2011. The confirmation must clearly show the benefits were received during 2010 or 2011. Missouri benefits, for example, you may turn in your Action Notice which has the months your received the benefits or contact social services and request written confirmation. If you didn't receive food stamp benefits in 2010 or 2011 please click here and complete the form.
SDI12 - Incorrect Student Information
An issue with your name, date of birth, marital status, or Social Security number was reported on your FAFSA. Please review your Student Aid Report (SAR) comments at www.fafsa.ed.gov and make the necessary corrections.
SLD12 - Student Loan Discharge
The National Student Loan Data System (NSLDS) indicates that one or more of your federal student loans have been discharged. If you don't plan to receive a student loan, please disregard this message. If you would like to receive student loans you must complete the Student Loan Discharge form before you can receive additional federal student loans. SLF12 - Student Loan Default
- You are currently in DEFAULT on a federal student loan.
The National Student Loan Data System (NSLDS) indicates you are in DEFAULT on one or more federal student loans. You are not eligible to receive any federal student aid until you resolve any loan default(s).
To resolve your defaulted federal student loan(s) contact the school associated with the loan or call the Federal Student Aid Center for assistance at 1.800.4FED.AID (1.800.433.3243).
SPI12 - Skipped Parent Information on FAFSA
You did not report information about your parents on your FAFSA. You also indicated you have a circumstance that requires you to follow-up with your Financial Aid Administrator before your eligibility for federal student aid can be determined. Please contact Student Services in person, by phone at 417.447.6900, or email financialaid@otc.edu and include in the subject line SKIPPED PARENT INFO ON FAFSA.
SSM12 - Student Signature Missing on FAFSA
- According to the FAFSA results received, you did not sign your FAFSA or any corrections submitted.
You must sign your FAFSA using your FAFSA PIN # online at http://www.fafsa.ed.gov/ or by signing the paper Student Aid Report (SAR). If you require assistance, please contact the Federal Student Aid Center at 1.800.4FEDAID (1.800.433.3243).
SSR - Selective Service Issue
- The Selective Service has reported you have not registered with them.
If you were born before 1960, registration is not required. If you are male or female and born during or after 1960, you must complete one of the following steps to update your FAFSA or financial aid file:
- If you are female, update Item 21 on your FAFSA to "Female" and submit the updated FAFSA.
- If you are male, update item 21 to "Male" and "Register Me" to Item 22 on your FAFSA and submit the updated FAFSA.
- If you have served in the military and have a DD214, submit a copy of the DD214 to the Financial Aid Office.

SUI12 - Student Untaxed Income
You must provide your income and spouse (if married) information in Items 35 through 39 and 44a through 44j on your FAFSA.
TVI12 - Tax and Income Inconsistency
- The amount either you or your parents' (if parents information was required on the FAFSA) reported for income tax is equal to or greater than the amount reported for the adjusted gross income.
- Student: review items 35 and 36 on the FAFSA and make the necessary corrections.
- Parent: review items 83 and 84 on the FAFSA and make the necessary corrections.
VAS12 - Veteran Status Issue
- The Department of Veterans Affairs (VA) could not confirm you are or will be a qualifying veteran for the purpose of receiving federal student aid.
If you believe you are or will be a qualifying veteran, contact your financial aid office and provide a copy of your DD214 (military separation form). If you are currently serving on active duty in the U.S. Armed Forces for purposes other than training, change your answer to Item 50 on your FAFSA from "Yes" to "No" and answer "Yes" to Item 49. If you are not and will not be a veteran, you must change the answer to Item 50 on your FAFSA from "Yes" to "No" and provide parental information, including the signature (PIN) of at least one of your parents on your updated FAFSA online.
