Financial Aid Notifications

- How do I use this page?

This page provides a list of codes (i.e. AON10, IVL10, DVW10, etc.) outlined in the Financial Aid Notification letter you received from the Financial Aid Office. 

Step 1: Refer to your letter, find your code(s) and click on the code(s) below that match those in your letter. 
Step 2:  Read and follow the directions provided in order to provide required information for your financial aid file.

If you should lose this message, you can still access what you need to know in your MyOTC account online at http://www.otc.edu:

  1. Click on Connect MyOTC and login
  2. Click on Access OTC tab at the top
  3. Click on Notifications under Financial Aid heading

AON11 - Aid Overaward Notice

  • Your FAFSA results indicate you have a previous overpayment of student aid from the Federal Pell Grant, Federal Stafford Student Loan Program, Federal SEOG or Federal Perkins Loan Programs.  Academic Competitiveness Grant, National SMART Grant, TEACH Grant, Iraq/Afghanistan Service Grant. 
  • To resolve your Federal Pell Grant overpayment, call the U.S. Department of Education at 1-800-621-3115, or write to the U.S. Department of Education, P.O. Box 5609, Greenville, Texas 75403-5609.
  • You may visit the NSLD website to review your overaward payment by clicking here.

 

BAD11 - Aid Denied/SAP Status

  • The College is unable to award financial aid at this time because our records show you have not met OTC Financial Aid Satisfactory Academic Progress (SAP) standards.

Please see the Financial Aid Satisfactory Progress Policy for further information.

If you believe your inability to meet SAP standards is appropriate for review, please contact Student Services in person, by phone at 417.447.6900, or email financialaid@otc.edu and include in the subject line Appeal Form.

BDF11 - Bachelor Degree on FAFSA

  • According to the information you reported on the FAFSA, you have completed a Bachelor's degree. Therefore, you are ineligible for the Federal Pell grant.

If you completed this item on the FAFSA in error, you may update your FAFSA online. If you have completed a Bachelor's Degree and desire to receive student aid, you must contact the OTC Financial Aid Office in person, by email at financialaid@otc.edu or phone 417.447.6900 for assistance.

CSI11 - Cash, Savings and Investment Form

  • Completed and signed Cash, Savings & Investments form.

DVL11 - Dependent Verification

  • Your financial aid file has been selected for a process called verification by the U.S. Department of Education.

The verification process requires the college to collect additional documents from the student and his/her family in order to verify information provided on the FAFSA.

You have been classified as a dependent student based upon information you provided on your FAFSA.

In order to most efficiently process your file, any worksheets, tax forms, W-2 forms or other documents submitted to the Financial Aid Office must be submitted at the same time include your OTC student ID# and name on each page you submit to the Financial Aid Office. If you are mailing or bringing the documents to the college, please DO NOT staple the documents together.

You may submit the documents:

  • In person at any of the college's locations
  • by fax to 417.447.6938
  • via email to financialaid@otc.edu with the email subject line VERIFICATION DOCS - OTC ID#

DVW11 - Dependent Verification Worksheet

ENS11 - Eligible Non-Citizen Status

  • The Department of Homeland Security (DHS) and/or Social Security Administration (SSA) were unable to confirm your eligible non-citizen status or U.S. Citizenship.

You must submit proof to the OTC financial Aid office you are an eligible noncitizen or a U. S. Citizen.  If you do not submit proof within 30 days you may not be eligible for federal student aid.

BNK11 - Active Bankruptcy Status

The National Student Loan Data System (NSLDS) indicates you have one or more student loans in an active bankruptcy status. Before you can receive additional federal student loans, you must contact the financial aid office at your school.  You may visit the NSLDS website to review your loans by clicking here.

SLD11 - Student Loan Discharge

The National Student Loan Data System (NSLDS) indicates that one or more of your federal student loans have been discharged.  Before you can receive additional federal student loans, you must contact the financial aid office. You may visit the NSLDS website to review your loans by clicking here.

HSG11 - High School/GED transcript missing

  • You indicated on your FAFSA you have not completed at least a high school diploma or GED, and the College does not have an official high school transcript or GED on file for you.

In order to be eligible for Federal Student Aid, you must have completed a high school diploma or GED.  If you have completed this requirement, you should update item 26 on your FAFSA and resubmit it, also send you final GED scores or have your high school send your official transcript to the OTC Admissions Office.

IDQ11 - Ineligible Drug Question

  • You are not eligible for federal financial aid based on your response to the question regarding a drug conviction.

You reported in Item 23 you are not eligible for federal student aid as a result of a drug-related conviction, or you do not know if your conviction(s) affect your eligibility. However, you may still be eligible to receive state, school, or other non-federal student aid. If you have answered this question incorrectly, you must correct Item 23 by updating your FAFSA online. You can also correct this item or get help with this question by calling 1-800-4-FED-AID (1-800-433-3243).

IDR11 - Independent Document Request

  • Your dependency status on the FAFSA requires validation.

Based upon your response on the FAFSA, you answered yes to one or more of the following conditions:

  1. Both your parents are deceased or you were in foster care/ward of the court since you turned age 13.
  2. You are an emancipated minor as determined by a court in your state of legal residence.
  3. You are in legal guardianship as determined by a court in your state of legal residence.
  4. You were determined as an accompanied youth who was homeless or were self supporting and at risk of being homeless on or after July 1, 2009 by:
    • Your high school or school district homeless liaison.
    • The director of an emergency shelter or transitional housing program funded by the US Dept of Housing and Urban Development.
    • The director of a runaway or homeless youth basic center or transitional living program.

    In order to verify your response, the OTC Financial Aid Office requires a legal court document that validates your status. If your date of birth was before January 1, 1987, please contact the Financial Aid Office in order for us to update your file accordingly.

IPI11 - Incorrect Parental Information

  • An issue with your parent's name, date of birth (DOB) or Social Security number (SSN) was reported on your FAFSA.

Please review your Student Aid Report (SAR) comments and make the necessary corrections.  You may contact the college at 417-447-6900 or email financialaid@otc.edu.

 

IVL11 - Independent Verification

  • Your financial aid file has been selected for a process called verification by the U.S Department of Education.

NDS11 - Non-Degree Seeking

  • Records at OTC indicate you are not seeking a degree or certificate.

Federal regulations require a student to be seeking a degree or certificate in order to be eligible to receive Federal Student Aid. In order to declare a degree or certificate, you may do so in person by completing the Program Update/Change form and submitting it to the college. Once you have updated your program, please notify us via email financialaid@otc.edu with the email subject line DEGREE SEEKING - OTC ID# so your financial aid file can be updated accordingly.  Failure to do so will delay processing of your financial aid.

PAE11 - Potential A+ Elig/transcript

  • Your A+ High School transcript is not on file

Your A+ High School transcript is not on file. You indicated on your OTC Admissions application you are an eligible A+ student who graduated from a Missouri A+ high school. In order to validate your A+ Program eligibility, the college is required to have your final official A+ designated high school transcript on file. Please contact the Missouri high school you graduated from and request the official A+ designated transcript be sent to the OTC Admissions Office.

If you graduated from high school before 2006, did not graduate from a Missouri high school or you are otherwise not an A+ Program student, please contact Student Services at any of the OTC campus sites for assistance with updating your records accordingly.

PAF11 - Potential Aid Fraud

  • You have one or more student loans that may have been obtained fraudulently.

When the Department of Education processed your FAFSA it was discovered you have one or more student loans that may have been obtained fraudulently. Therefore, you are not eligible to receive any federal student aid until this issue is resolved. You are encouraged to contact the Federal Student Aid Center at 1.800.4FED.AID (1.800.433.3243) for assistance.

PIT11 - Potential Identity Theft

  • The National Student Loan Data System (NSLDS) indicates you have one or more student loans in a status associated with identity theft.  Before you can receive additional federal student loans, you must contact the financial aid office at your school.  
  • You may visit the NSLDS website to review your loans by clicking here.

PFT11 - Parent(s) 2010 Federal Tax Return

  • A signed copy of your parent(s) 2010 Federal Income Tax Return

You must submit a signed copy of your parent's 2010 Federal Income Tax Return. Be sure your OTC ID number is in the upper right corner of each page submitted. If you do not have a copy of the return, you may request a tax transcript from the IRS by calling 1.800.829.1040. Be sure the document is signed before submitting to the Financial Aid Office.

PSM11 - Parent Signature Missing

  • According to the FAFSA results received, your parent(s) did not sign your FAFSA or any corrections submitted.

You must have at least one parent sign your FAFSA online at http://www.fafsa.ed.gov/ using their parent FAFSA PIN or by signing the paper Student Aid Report (SAR).

PW211 - Parent(s) 2010 W-2 Statement

  • Submit parent(s) 2010 W-2 wage and earnings statements. ONLY ONE W-2 PER PAGE on 8 1/2 x 11 paper.

SCF11 - Special Circumstance - FAFSA

  • You did not report information about your parents on your FAFSA.

Based upon the FAFSA information, you did not report information about your parents on your FAFSA. You indicated you have special circumstances that will require consultation with a Financial Aid Advisor. Please contact Student Services in person, by phone at (417)447-6900, or email us at financialaid@otc.edu to schedule an appointment.

SDI11 - SSN/DOB Inaccurate

  •  An issue with your name, date of birth (DOB) or Social Security number (SSN) was reported on your FAFSA.

Please review your Student Aid Report (SAR) comments and make the necessary corrections.  You may contact the college at 417-447-6900 or email financialaid@otc.edu.

SFT11 - Student 2010 Federal Tax Return

  • A signed copy of your 2010 Federal Income Tax Return

You must submit a signed copy of your 2010 Federal Income Tax Return. Be sure your OTC ID number is in the upper right corner of each page submitted. If you do not have a copy of your return, you may request a tax transcript from the IRS by calling 1.800.829.1040. Be sure the document is signed before submitting to the Financial Aid Office.

SLF11 - Student Loan Default

  • You are currently in DEFAULT on a federal student loan.

The National Student Loan Data System (NSLDS) indicates you are in DEFAULT on one or more federal student loans. You are not eligible to receive any federal student aid until you resolve any loan default(s).

You may visit the NSLDS website to review your loans and determine what loans are in default by clicking here.

To resolve your defaulted federal student loan(s) contact the school associated with the loan or call the Federal Student Aid Center for assistance at 1.800.4FED.AID (1.800.433.3243).

SSM11 - Student Signature Missing on FAFSA

  • According to the FAFSA results received, you did not sign your FAFSA or any corrections submitted.

You must sign your FAFSA online (http://www.fafsa.ed.gov/) using your FAFSA PIN or by signing the paper Student Aid Report (SAR). If you require assistance, please contact the Federal Student Aid Center at 1.800.4FEDAID (1.800.433.3243).

SSR11 - Selective Service Registration

  • The Selective Service has reported you have not registered with them.

If you were born before 1960, registration is not required. If you are male or female and born during or after 1960, you must complete one of the following steps to update your FAFSA or financial aid file.:

  1. If you are female, update Item 21 to "Female" and submit the updated FAFSA.
  2. If you are male, update item 21 to "Male" and "Register Me" to Item 22 on your FAFSA.
  3. If you have served in the military and have a DD214, submit a copy of the DD214 to the Financial Aid Office

If you believe you have are exempt, please contact the Selective Service at 847-688-6888 or at www.sss.gov.

SW211 - Student 2010 W-2 Statement

  • Student's 2010 W-2 earnings statements from wages.  ONLY ONE W-2 PER PAGE on 8 1/2 x 11 paper.

TIV11 - Tax and Income Inconsistency

  • The amount either you or your parents' (if parents information was required on the FAFSA) reported  for income tax is equal to or greater than the amount reported for the adjusted gross income.
  • Student:  review items 35 and 36 and make the necessary corrections. 
  • Parent:  review items 83 and 84 and make the necessary corrections.

VAS11 - VA Status

  • The Department of Veterans Affairs (VA) could not confirm you are or will be a qualifying veteran for the purpose of receiving federal student aid.

If you believe you are or will be a qualifying veteran, contact your financial aid office and provide a copy of your DD214 (military separation form). If you are currently serving on active duty in the U.S. Armed Forces for purposes other than training, change your answer to Item 50 from "Yes" to "No" and answer "Yes" to Item 49. If you are not and will not be a veteran, you must change the answer to Item 50 from "Yes" to "No" and provide parental information, including the signature (PIN) of at least one of your parents on your updated FAFSA online.

XFT11 - Spouse 2010 Federal Tax Return

  • Submit a signed copy of your spouse's 2010 Federal Income Tax Return.

If you do not have a copy of the return, you may request a tax transcript from the IRS by calling 1.800.829.1040. Be sure the document has a handwritten signature before submitting to the Financial Aid Office.

XW211 - Spouse 2010 W-2 Statement

  • Submit spouse's 2010 W-2 earnings statements from wages.  ONLY ONE W-2 PER PAGE on 8 1/2 x 11 paper.
